What Goes WrongBetween Service Visits
Anonymised accounts from systems running under monitoring. Each one is a failure that every automated check passed, found only because a second independent measurement disagreed with the first. No customer names, no site identifiers.
Fouled conductivity probe
The controller said 1,000. The water said 1,500.
A commercial cooling tower. The probe fouled while sitting at setpoint, so no alarm fired and blowdown never triggered. Conductivity climbed 500 µS/cm over about a week and scale formed on the fill before anyone saw it.
Biocide feed verification
An ORP spike is one signal. Verification takes four.
A rising ORP trend after a scheduled feed looks like proof that the biocide fed. It confirms one thing and assumes three others: that the pump ran, that chemical left the tank, and what residual the system actually reached.
These are anonymised accounts of real systems. Customer names, site identifiers and account details are deliberately absent, and will stay absent.
